Activity Registration Information

The online registration form must be completed for all MSHSL school activities (7-12 grade).

The form only needs to be filled out ONCE a year. To add activites/sports AFTER you have filled out the registration, please email ebelden@lfalls.k12.mn.us. DO NOT fill out another form.

Athletic Registration: Needs to be filled out for ALL 7-12 sports. (6th grade registers with Community Services)

Activities Registration: Needs to be filled out for Band, Choir, One-Act Play, Speech and Visual Arts. (**If you are in a sport, you can add these activities to that form**)

*Please use your student's FULL NAME when registering no nicknames
*You can choose multiple sports/activities per season by pressing the "CTRL' key and highlighting the next selection on the form.
*IMPORTANT: if you are a TRANSFER STUDENT, please see Mr. Jordan, AD prior to registering to discuss eligibility.
Student ID = first 5 letters of last name, first 3 of the first and 000 (mousemic000)

**Fees will be entered into your childs Skyward account within 72 hours after form is completed.**
